Deed of tenancy by the entirety or Joint tenancy with right of survivorship in most states

Life estate deed can do the same thing.

The will of the deceased person.

You need to create a deed on death there are only 10 states that allow it. They are commonly called Transferred on Death Deeds TODs

The deed doesn’t do anything, it’s how title is taken on the deed that matters. Joint tenancy is what you need.
